Central retinal vein occlusion (CRVO) is a severe eye condition characterized by the blockage of the main vein that drains blood from the retina. This blockage disrupts the normal flow of venous blood, leading to symptoms such as sudden, painless vision loss and a "curtain-like" effect over the eye. CRVO is commonly associated with systemic diseases like hypertension, diabetes, and atherosclerosis, as well as local factors such as glaucoma or inflammation within the eye.
Clinical examination typically reveals signs such as retinal hemorrhages, dilated and twisted veins ("blood and thunder" appearance), cotton wool spots, and swelling of the optic disc. Management strategies focus on addressing underlying systemic conditions, if present, and managing complications like macular edema with targeted treatments such as intravitreal injections of anti-VEGF drugs or steroids. Laser therapy may be considered for cases where the retina shows signs of severe ischemia. Timely diagnosis and intervention are critical in preventing permanent vision loss in CRVO patients.
