Title : Unpredictable outcomes post phakic intraocular lens implantation
Abstract:
Purpose: Phakic IOL implantation is a viable option for refractive errors with promising visual outcomes. However, despite meticulous pre-operative planning and calculations, one may encounter unpredictable outcomes post phakic IOL implantation. At such times, it becomes difficult to decide whether to intervene or not. We hereby present a compilation of such cases and their management.
Method: Records of all patients who underwent phakic IOL implantation in our refractive department were reviewed. Cases with early postoperative complications like decreased visual acuity, raised IOP, or abnormal AS-OCT findings were included. Patients needing further surgical intervention post-implantation were also analysed.
Result: Among the eyes that underwent phakic IOL implantation, postoperative complications included residual refractive error, malposition requiring re-orientation, and issues related to vaulting (both high and low). Some eyes required further surgical intervention such as re-rotation or exchange with a larger IOL, while others could be managed conservatively.
Conclusion: Meticulous and accurate measurements are needed while planning a phakic IOL procedure. Despite best efforts one may land up into unpredictable outcomes.
