Title : Reconstructive eyelid surgery after trauma with Lacrimal canalicular repair
Abstract:
Introduction: Traumatic eyelid injuries often result in cicatricial scarring, which not only distorts the natural eyelid contour but also impairs its protective and functional roles. Involvement of the lacrimal drainage system further complicates the condition, causing persistent epiphora, ocular surface irritation, and cosmetic disfigurement.
Purpose: To present a clinical case of reconstructive eyelid surgery following trauma with simultaneous lacrimal canal repair, emphasizing both functional and cosmetic outcomes.
Methods/Materials: A 19-year-old male presented with sequelae of childhood trauma (sustained at the age of 4 after a horse kick) that caused laceration of the medial upper eyelid with disruption of the lacrimal canal. Initial wound management was performed at the time of trauma; however, residual eyelid deformity and functional complaints persisted. The patient underwent reconstructive eyelid surgery using autologous tissues combined with silicone stent intubation of the lacrimal system.
Results: The surgical intervention restored eyelid anatomy, improved lacrimal drainage, and achieved complete eyelid closure. Postoperative follow-up demonstrated resolution of epiphora, improvement of ocular surface comfort, and satisfactory cosmetic appearance.
Conclusion: Reconstructive eyelid surgery combined with lacrimal canal repair provides both functional and aesthetic rehabilitation in patients with traumatic eyelid deformities. The use of autologous tissues with silicone stenting ensures long-term patency of the lacrimal system while maintaining eyelid integrity. This case highlights the importance of individualized surgical planning in managing complex eyelid trauma.
