Title: Locked pupil clouded lens: Clinical profile and surgical outcomes of posterior synechiae in complicated cataract
Abstract:
A case of a 28-year-old woman, housewife, with decreasing vision on her Left Eye (LE) since 7 years ago. Other complaints like red eyes, watery, and glare when exposed to light were not found. No history of trauma. History of eye infection was found, not treated properly, only using on-the-counter eye drops. History of teeth infection and cavity was found from 7 years ago until now, without regular dental check-ups. Best Corrected Visual Acuity (BCVA) preoperatively of the LE were light perception only. Intraocular Pressure (IOP) preoperatively of the LE was 16 mmHg. Under slit lamp for the anterior segment, LE had small pupil with adhesion between iris and anterior lens 360 degree irregularly and the lens looked cloudy. Flare cell and keratic precipitate were not found. For the RE, the examination was within normal limits. Red green perception was normal on both eyes. Based on the history and physical examination, the patient was diagnosed with Oculi Sinistra Complicated Cataract with Posterior Synechiae. Phacoemulsification with Intraocular Lens (IOL) implantation and release of posterior synechiae was done on her LE. Last follow-up postoperatively showed a significant improvement of visual acuity on her LE from light perception preoperatively to 0.1. In the first postoperative follow-up, the IOP increased to 30 mmHg, then decreasing back to normal at 14 mmHg in the second follow-up. Post-operative treatment was given topical combination of dexamethasone, neomycin sulfate, and polymyxin B sulfate 6x1 eye drops daily on her LE, Cefadroxil 2x500 mg orally, and Paracetamol 3x500 mg orally if necessary. Posterior synechiae significantly contribute to the adversity of cataract surgery and visual outcomes in patients with complicated cataract. Appropriate intraoperative management and postoperative evaluations are essential to optimize the outcomes.
